About This Survey

History: About every 5 years or so during the past 70 years, we invite a representative group of our customers from across Michigan to participate in a home and appliance characteristics survey. You have been randomly selected to be invited to participate this year.

Participation is Voluntary: Your participation is completely voluntary and as a small token of appreciation for your time if you do choose to participate, you will be entered into a sweepstakes to win one of ten $200 electronic gift cards according to the official rules below. Your input helps us design our rebates and programs to be of value to you and other customers like you for things like efficient appliances and other home and apartment improvements needed to help customers save energy, money and improve home comfort.

Confidentiality: We are sending this letter directly to you with your address on the survey to indicate the dwelling/residence we are asking about as some customers have more than one dwelling/residence. However, when we process your data, we will make it anonymous and not link it to you or your account specifically and not use it for targeted marketing or sales. Your answers will be combined with answers of other participating customers across Michigan and used to track changes and trends across the state overall and for larger subgroups of customers such as those in a specific region, county, or age group.

Returning the Survey: Once your paper survey is complete, you may return it in the provided postage paid envelope, addressed to PO Box 19, Hazel Park, MI 48030-9902. Or if you wish to complete the survey online, you may go to the following website: ConsumersEnergy.com/RASS2025.This website is only for recipients of the paper survey packet. Please only complete one survey per household, either the paper survey or the online survey.

2025 Home and Appliance Survey Official Rules
NO PURCHASE NECESSARY TO WIN. PURCHASING WILL NOT INCREASE YOUR ODDS OF WINNING.

  1. Eligibility: The Home and Appliance Survey Sweepstakes (the “Sweepstakes”) is open to participants (the “Entrants”) who complete and submit the Home and Appliance Survey (the “Survey”). By taking the Survey, you agree to these Sweepstakes rules (the "Official Rules"). Entrants who are employees, officers, directors and agents of Consumers Energy, its parent company, affiliates, subsidiaries, advertising, promotion and fulfillment agencies, and program implementation contractors and persons living in their same household are not eligible to participate. The Sweepstakes is subject to all applicable federal, state, and local laws and regulations. Void where prohibited by law.

  2. Sponsor: The Sweepstakes is sponsored by the Consumers Energy Company, One Energy Plaza, Jackson, MI 49201.

  3. Timing: Sweepstakes will be held on or around February 28, 2025, after data collection is complete. The opportunity to enter the Sweepstakes will begin on the first day of receiving the survey through the last day of fielding (the "Sweepstakes Period").

  4. Prizes: Ten (10) $200 electronic gift cards from Tangocard.com will be awarded at random to ten (10) Entrants. No transfer, assignment, cash redemption, or substitution of prize may occur except by Consumers Energy at its sole discretion due to prize unavailability or causes otherwise determined solely by Consumers Energy. All taxes, if any, are the winner's sole responsibility. In no event will there be more prizes awarded than are stated in these Official Rules. Any difference between the stated value and actual value will not be awarded. Lost or stolen prizes will not be replaced. Winners agree to accept the prize “as is”, and Entrants hereby acknowledge that The Sponsor has neither made nor is in any manner responsible or liable for any warranty, representation, or guarantee, expressed or implied, in fact or in law, relative to the prize, including express warranties (if any) provided exclusively by a prize supplier that are sent along with the prize.

  5. How to Enter: Entrants can submit their Entry by completing and submitting the Survey as instructed by the Sponsor. By submitting the Survey, you indicate your agreement and compliance with the Official Rules. Your Entry must be in accordance with the specific requirements listed below. Successful Entry according to the above “How to Enter” rules automatically result in Entry into the Sweepstakes.

  6. Sweepstakes Restrictions: Consumers Energy may disqualify any entry that it determines, in its sole discretion, has failed to conform to any of these Official Rules. Entrant must be a Consumers Energy account holder at the time of the Survey or household member of a household with a Consumers Energy account at the time of the Survey. Only one entry per household.

  7. Winner Determination: Following each Sweepstakes Period, a random drawing will be held with ten (10) winners selected for each prize. In early March 2025, the ten selected winners will receive an email invitation directly from Tangocard.com with the subject line “Thanks for taking our survey!”. The invitation will allow winners to select an electronic gift card from a broad range of major brands, or it can be donated to a charity organization. If a selected winner cannot be contacted, does not comply with these Official Rules, does not provide an address to which to send the prize, or the prize is returned as undeliverable, such selected winner forfeits the prize, and a new winner will be randomly chosen in compliance with these rules. In the event the second winner forfeits the prize, any unawarded prizes will remain unawarded.

  8. Disqualification, Modification and Cancelation: Any Entrant who attempts or encourages, either directly or indirectly, the entry of multiple or false contact information under multiple identities, or uses any device, service or artifice to enter false Entries, as determined by the Sponsor, will be disqualified and any and all prizes to which the Entrant may have been awarded or entitled will be canceled.

  9. Odds of Winning: Odds of winning a prize depend on the number of eligible Entries.

  10. Privacy: Entry into this Sweepstakes requires the Entrant to provide certain personal information upon entry into the Sweepstakes. Entrants are informed that the personal data collection in connection with this Sweepstakes will be collected by Sweepstakes engine and reside on United States based servers and will therefore be subject to applicable United States laws and Sweepstakes Entities privacy policies located at https://www.consumersenergy.com/privacy.

  11. Limitations of Liability: By participating, Entrant releases Consumers Energy and its Subcontractors, officers, directors, shareholders, and employees from any and all liability for any claims, costs, injuries, losses or damages of any kind caused by participation in the Sweepstakes, including unauthorized or illegal access to personally identifiable or sensitive information or acceptance, possession, use, misuse, or nonuse of the prize that may be awarded; acknowledges that said parties have neither made nor are in any manner responsible or liable for any warranty, representation or guarantee, expressed or implied, in fact or law, relative to the prize; and waives any rights to claim ambiguity with respect to these Official Rules.

  12. Disputes: This Sweepstakes is governed by the laws of the State of Michigan, without respect to conflict of law doctrines. As a condition of participating in this Sweepstakes, you agree that any and all disputes which cannot be resolved between the parties, and causes of action arising out of or in connection with this Sweepstakes, shall be resolved individually, without resort to any form of class action, exclusively before a court located in Michigan having jurisdiction. Further, in any such dispute, under no circumstances will Entrants be permitted to obtain awards for, and hereby waive all rights to claim punitive, incidental or consequential damages, including attorneys' fees, other than Entrant’s actual out-of-pocket expenses (e.g., costs associated with entering). Entrants further waive all rights to have damages multiplied or increased. In the event of a dispute about the identity of an Entrant, Consumers Energy will issue the award to the account holder at time of the Sweepstakes. The account holder is deemed to be the person in whose name the Consumers Energy account is registered for the household. Failure to comply with these Official Rules may result in disqualification from the Sweepstakes.
